9am-12pm June 23-25 2025

STUDENT REGISTRATION

Ages 4-13 years old are welcome to join us for Bible stories, games, projects, music, and more!
We will have 3 age groups: 4-5 year olds, 6-9 year olds, and 10-13 year olds in their respective age groups.
Lunch is served at 12PM upon dismissal.
If you have more than one student, you will need to sign a form for each student!

MEDICAL RELEASE &
STUDENT BEHAVIOR AGREEMENT

Medical Release

If you or your child should suffer an injury while participating, you will be personally responsible for your medical or injury related expenses.

I also give permission to the designated adult supervisor in charge to secure emergency medical treatment for the minor(s) names above. I also agree to hold NSCC and/or their assignees, harmless in the event of an injury or accident.

Behavior Contract

I, the student, agree to the following:

To respect all teachers, leaders, parents, staff and NSCC facility representatives at all times

Use language and actions that are honorable, inclusive, and considerate of others.

Treat property and equipment of the facility with care.

Dress modestly and appropriately.

Stay onsite New Song Christian Community Church areas at all times.
